PATIENT EDUCATION

Patients receive a brief demonstration of the COPD Tel-Assist during their visit to the clinic. Patients learn the importance of interacting with the system and that nurses are monitoring it.

REACHING OUT TO PATIENTS

The COPD Tel-Assist System contacts patients regularly using their preferred mode of contact - emails, text messages and automated voice calls. The schedule of contacts is calibrated to the specific needs of each patient.

PATIENTS CAN CALL IN

Patients are encouraged to call inbound to the system when they feel their symptoms worsening or anytime they wish to speak with a nurse manager.

The moment a patient reports an issue to the COPD Tel-Assist System a member of the care team is alerted by text message and/or secure email.
